Evoke can integrate with over ___#?____ EHR/EMR systems. As part of the Evoke Solution, Evoke clients who use our software and service pay no upfront charge, nor do they pay extra if adjustments to the software are needed later. Nothing cookie-cutter here.
Patient Benefits
The use of cardiac devices is fairly routine for cardiologists and clinics. But at Evoke, we realize that for patients, it’s very personal and can be concerning when they or a loved one receives a pacemaker, defibrillator, or cardiac monitor. With Evoke, patients can have confidence knowing they are in the hands of experienced clinicians—skilled and trusted experts who know what to look for and will relay any alerts to clinic staff immediately.
Your patient is always our first priority. The Evoke Solution offers patients several benefits, including:
Convenience
Remote cardiac device monitoring or remote patient monitoring eliminates the need for a patient to take time away from work or family; no coordinating transportation to the physician’s office; and no need to arrange for an accessible van or assistance transporting a patient from a skilled nursing facility. Not only is this more convenient, it also reduces exposure to other illnesses. With RPM, the patient can remain in the comfort of their home during transmission of data.
Ownership of one’s health
Smart phones and watches that monitor health data have raised the public’s interest in knowing their wellness status. This helps with compliance. With Evoke, patients and their families have the reassurance of knowing that an experienced medical team is ensuring their device is working properly and their condition is constantly being monitored. The more self-aware, the more likely a patient is to make small adjustments to their daily routine.
Assistance with set-up and ongoing transmissions
The Evoke Solution helps patients, physicians, and the clinical team live and work better. Evoke contacts patients to assist with the setup of equipment, its ongoing use, and makes sure the transmission schedule is clearly understood and followed.
Here’s how the process works:
Step One
Sally’s physician informs her she will be getting a pacemaker.
Step 2
Sally goes to the hospital and the pacemaker is successfully implanted. Evoke is informed of the implanted device and receives her demographic information.
Step 3
Within a day or two, an Evoke representative calls Sally to help her set up her device for monitoring. This involves giving Sally step-by-step instructions on unboxing and setting up her monitor, and making sure she is connected.
Step 4
Evoke schedules Sally’s routine transmissions, per guidelines, that will take place throughout the year. (Current guidelines stipulate pacemakers and ICDs should be checked every 91 days. Heart Failure and loop recorder devices should be checked every 31 days.)
Step 5
Evoke handles all interactions with Sally, answering any questions. Between schedule checks, if there is an alert or any problem with her device, Evoke informs the clinic. Sally can rest easy knowing that her device is monitoring her health 24/7/365.
Step 6
For each transmission, Evoke’s expert clinicians analyze the report and seal it so it cannot be edited. They then store it in the practice’s electronic healthcare record for the provider to review and sign.
