Implantable Devices
At Evoke, we built our software and service around implantable cardiac devices. These devices relay information to the healthcare provider such as heart rhythm changes, device malfunction and fluid retention in the lungs. Programmable alerts can be sent between regularly scheduled follow-ups. This allows problems to be detected and treated much earlier than would be possible without remote monitoring. Remote patient monitoring devices are used to collect health data including heart rate, vital signs, blood pressure, and electrocardiograms. Cardiac Monitoring
A Holter monitor is a small, wearable device that records the electrical activity of the heart continuously over 24 hours or longer, acting as a portable electrocardiogram (ECG). It can detect or determine the risk of irregular heartbeats (arrhythmias), heart palpitations, or unexplained dizziness. A Holter monitor test may be done if a traditional electrocardiogram (ECG or EKG) doesn't provide enough details about the heart's condition. A long-term Holter can be worn and monitored after 10 days, 3 month, and 6 months.
This method offers key advantages: analysis of each heartbeat without disrupting a patient’s routine, and the ability to immediately enact an emergency response if any abnormalities are detected. This not only gives the physician immediate, relevant data, it also offers peace of mind to the patient and ultimately, more efficient care. The MCT device is also able to record patient-activated or symptomatic tests, where the patient is able to manually push a button and enter the symptoms they are feeling. The device also auto-triggers for bradycardia, tachycardia, pauses, or atrial fibrillation (AFib).
Chronic Care Management
This approach helps patients with chronic diseases and certain health conditions understand their illnesses and stay engaged in therapies and treatment so they can achieve a positive quality of life. RPM plays a role by giving patients insight into their condition and encouraging them to pay attention to changes in their health and adhere to the transmission schedule. Remote Therapeutic Monitoring
Remote Therapeutic Monitoring services monitor health conditions, including musculoskeletal system status, respiratory system status, therapy (medication) adherence, and therapy (medication) response. RTM is intended for the management of patients utilizing medical devices that collect non-physiological data. The data can be self-reported as well as digitally uploaded.
